There’s something wrong with the questionnaire. Whenever I finish it , there is a message showing a missing answer although I finish all of them except part 4 I finish it

Although I answered question 1 in the subject questionnaire, it doesn’t submit my answers

Any way, I COMPLETED the four parts
The first three parts, question 1 is marked by a star as it is not answered and requires one but I did many times and I tried to add two answers but the same message.
The last part is ok
